QUALIFICATIONS
Education & Experience * Care Coordinator I: * High School Diploma or equivalent AND 2 years’ experience providing community engagement, community resources/linkage, or direct service support to youth & families * AA Degree in social service, psychology, juvenile justice, sociology, child development or health/human services related field AND 1 year experience providing community engagement, community resources/linkage, or direct service support to youth & families * Care Coordinator II: * Bachelor’s degree in social service, psychology, juvenile justice, sociology, child development or health/human services related field AND 1 year experience providing community engagement, community resources/linkage, or direct service support to youth & families * Spanish Speaking PreferredESSENTIAL JOB FUNCTIONS
Position Specific * Assess client needs in the areas of physical health; mental health; SUD; oral health; trauma- informed care; social supports; housing; vocational/employment; wellness; and referral and linkage to community-based services and supports. * Collaboratively develops and implements the Individual Care Plan/Health Action Plan * Offer services where the client lives, seeks care, or finds most easily accessible, including office-based, telehealth, or field-based services. * Connect clients to other social services and support that are needed (e.g., community support group). * Coordinates/advocates on behalf of the client with health care professionals (e.g., Primary Care Physician, Health Specialists, Mental Health Providers, etc.) * Utilize evidence-based practices, such as Motivational Interviewing, Harm Reduction Techniques, and Trauma- Informed Care principles.- Work collaboratively with hospital staff regarding Transitional Care Planning
- Conduct outreach and engagement activities to facilitate linkage to the ECM
- Accompany clients to office visits, as needed and appropriate.
- Evaluate progress and update goals.
- Arrange transportation
- Complete all documentation within the timeframes established by the
- Attend weekly staff/team meetings and supervision.
- Attend training as assigned (e.g., ACEs Certification)
- Participate in on-going training to expand and develop professional skills
- Perform other duties as necessary for the agency, as assigned
- Typical hiring range: $24.00 – $28.00 per hour
- Full salary band: $24.00 – $38.00 per hour
- Typical hiring range: $28.00 – $33.00 per hour
- Full salary band: $28.00 – $40.00 per hour
