Description

• Conduct thorough evaluations for patients in both inpatient and outpatient environments, crafting personalized treatment plans
• Deliver physical therapy interventions centered around mobility, strength, balance, and overall function
• Engage with nurses, doctors, case managers, and other rehabilitation specialists to maximize patient outcomes
• Guide patients and families through therapeutic objectives, discharge planning, and home exercise programs
• Maintain detailed records of evaluations, treatments, and patient progress in line with facility standards and regulatory requirements
• Participate actively in team rounds and interdisciplinary meetings to facilitate coordinated care

Education

• Doctor of Physical Therapy (DPT) or similar qualification from an accredited institution

Certifications

• Current state licensure as a Physical Therapist
• CPR certification is advantageous
• Specialty certifications (e.g., neurological, geriatric) are beneficial, but not mandatory

Skills

• Previous clinical experience is highly preferred
• Capability to work with patients facing complex medical and mobility challenges
• Excellent communication and teamwork abilities
• Proficiency in digital documentation and clinical charting systems
