Key Responsibilities\n

  • Utilize your education and clinical expertise to conduct thorough psychosocial assessments and formulate independent diagnoses for patients in our hospice care program.

  • Develop and implement patient-centered treatment plans in partnership with patients and their families, focusing on their specific needs and preferences.

  • Employ evidence-based therapeutic techniques, such as solution-focused therapy, behavioral activation, and motivational interviewing, to facilitate positive behavioral change and emotional support.

  • Coordinate with interdisciplinary team members, including other clinicians, psychologists, and primary care providers, to ensure an integrated approach to patient care.

  • Monitor patient progress towards treatment goals through regular follow-ups, utilizing both virtual and in-person check-ins as required.

  • Encourage adherence to psychotropic medication regimens as prescribed, actively assessing side effects and treatment effectiveness to provide informed feedback to prescribing providers.

This role primarily focuses on patients residing in hospice care settings, where the goal is to provide compassionate support for patients and their families during challenging times. You will work with individuals facing life-limiting illnesses, offering emotional, social, and practical assistance tailored to enhance their quality of life and that of their loved ones.

Qualifications\n

  • A Master's degree in Social Work from an accredited program.

  • A current and unrestricted license to practice independently as a Licensed Clinical Social Worker (LCSW) within the state of California.

  • At least 1 year of professional experience as an independently licensed therapist, preferably within a mental health or hospice environment.

Preferred Experience\n

Experience working with hospice or palliative care patients, as well as familiarity with treatment protocols for end-of-life care, will be considered an asset. Candidates who have utilized electronic health records (EHR) systems and case management software will have an advantage in this position.

Compensation and Benefits\n

We offer a competitive salary range of $120,000 to $140,000 annually, along with a sign-on bonus of $3,000. Our comprehensive benefits package includes an annual incentive bonus, health and dental plans, a 401(K) with company matching, reimbursement for license renewal fees, opportunities for continuing education, and malpractice insurance coverage.
