Responsibilities Deliver comprehensive, patient-centered primary care, including high-quality diagnostic and treatment services to improve community health. Coordinate seamless care transitions for patients, manage specialized consultations, inpatient admissions, and collaborate with case management for discharge planning. Lead or participate in continuous quality improvement initiatives to enhance efficiency and cost-effectiveness of care delivery within the clinic and wider system. Maintain complete and timely patient documentation and electronic medical records to ensure compliance and support financial integrity. Maintain autonomy in practicing and demonstrate comfort managing a solo outpatient primary care panel within a clinic environment. Minimum Requirements Licensure/Certification/Registration: Physician MD/DO credentialed from the Wisconsin Medical Examining Board prior to hire or transfer date required. Education: Doctor of Medicine (MD) or Doctor of Osteopathy (DO) required. Advanced Life Support preferred. American Heart Association or American Red Cross accepted.
