LPN/LVN: Skilled Nursing & Memory Care Leader

$27.53 - $38.27 per hour

Licensed Practical/Vocational Nurse (LPN/LVN) positions are available in assisted living, skilled nursing, and memory care. The role ensures quality care through monitoring staff, physician communication, care planning, and resident observation, while also providing direct patient care. Responsibilities Assist routine daily direction and monitor the work of nursing assistants, ensuring high standards of nursing care. Assist in the orientation of new LPN/LVNs and nursing assistants, reinforcing training standards. Explain nursing procedures to staff and provide assistance for ongoing understanding and training. Communicate regularly with the Nurse Manager or Director of Nursing regarding observations, suggestions, resident and employee activities, concerns, and needs. Observe and evaluate residents’ health, mental and emotional conditions and communicate significant changes to appropriate staff or physicians and family members as required. Provide direct patient care including, but not limited to, procedures such as catheter placement and care, colostomy care, suction, dressings, naso‑gastric tubes, and other treatments. Participate in reports at the beginning and end of shifts to ensure proper communication of resident information. Assist in the admission, orientation, and observation of new residents, ensuring physician orders for diets, therapy, medication, etc., are transcribed and communicated correctly. Qualifications Graduate of an approved school of licensed practical/vocational nursing. Current license in good standing as an LPN/LVN in the State. One year of nursing experience. Devotion to standards and professional nursing principles. Compensation Pay range: $27.53 – $38.27 per hour. The offered rate may vary based on qualifications, experience, skills, education, geography, and internal/external equity.
