Work Environment

This position is primarily onsite, delivering therapy services through home care settings across North Fort Myers, Florida. As a Physical Therapist, you will have the opportunity to work directly with patients in their personal settings, creating a unique and intimate therapeutic environment. This setup allows you to assess patient needs more accurately and tailor your treatment plans to their real-life situations.

Key Responsibilities

• Conduct thorough evaluations by reviewing medical histories, performing specific assessments, and understanding each patient's unique physical condition.
• Effectively communicate evaluation results and treatment plans to both patients and their families, ensuring active involvement in goal-setting and care decisions.
• Implement evidence-based treatment protocols tailored to the individual needs of your patients, adapting as necessary given their responses and changing health status.
• Collaborate with other healthcare professionals to address complex cases, documenting any changes in patient conditions that could necessitate adjustments in care or referrals.
• Oversee discharge planning and post-therapy care, ensuring that documentation is precise, timely, and aligns with all regulatory requirements, including comprehensive evaluation notes and treatment summaries.
• Demonstrate effective time management skills, maintaining a well-organized schedule to meet productivity goals while being flexible and responsive to patients' needs.
• Engage in departmental initiatives that promote community awareness and accessibility to physical therapy services, participating in marketing efforts when appropriate.
• Contribute to departmental goals and patient care objectives while adhering to local and national accreditation guidelines.
• Proactively pursue continuing education opportunities to enhance your professional skills and competencies, mentoring junior staff or students as needed.
• Foster an exceptional customer experience by being adaptable to various patient needs and creating a welcoming atmosphere.

Patient Population / Client Focus

The ideal candidate will work with a diverse patient population, including individuals recovering from surgery, those managing chronic conditions, and patients requiring rehabilitation due to injury or age-related issues. Your role is pivotal in facilitating their recovery journey and empowering them to regain independence in their everyday lives.

Qualifications

• A Doctorate in Physical Therapy (DPT) is required.
• Valid Physical Therapy license in the state of Florida.
• Current Certified Basic Life Support (BLS) certification from the American Heart Association or equivalent.

Preferred Experience

• A minimum of two years of relevant experience is preferred, although new graduates are also encouraged to apply, as we provide extensive support during the onboarding process.
• Experience in home care settings is a plus, providing familiarity with the challenges and rewards associated with at-home therapy.

Compensation and Benefits
