Work Environment

This role will be performed onsite, primarily in home care settings, providing you the unique opportunity to deliver care in a community that values personal interactions. You will be working Monday through Friday from 8:00 am to 5:00 pm, allowing for a balanced professional and personal life. Key Responsibilities

• Conduct thorough examinations and evaluations of patients, analyzing medical histories and performing specific assessments to establish a comprehensive treatment plan tailored to each individual's needs.

• Implement evidence-based treatment protocols and adjust strategies based on patient progress and evolving healthcare environments to achieve optimal outcomes.

• Document all patient interactions clearly and promptly, including initial evaluations, daily encounter notes, progress summaries, and discharge planning, ensuring compliance with state, local, and federal regulations.

• Engage in interdisciplinary consultations when necessary, recognizing changes in patient conditions that may require modification of treatment plans and additional input from other healthcare professionals.

• Participate in team meetings and contribute to initiatives focused on removing barriers to patient access, ensuring that every individual can benefit from our services.

• Actively engage in the continuous improvement of service delivery by monitoring and evaluating patient feedback and outcomes.

Patient Population / Client Focus

In this role, you will work primarily with patients who require physical therapy due to orthopedic conditions, post-operative recovery, and various functional limitations that affect their daily lives. Our clients are diverse, ranging from athletes recovering from sports injuries to seniors regaining mobility after surgeries or injuries. Your expertise will directly contribute to their physical rehabilitation and quality of life, making a significant impact on their journey to recovery.

Qualifications

• A Doctorate of Physical Therapy (DPT) from an accredited institution.

• A valid state license as a Physical Therapist (PT) from the Physical Therapy Examining Committee.

• A minimum of two years of outpatient orthopedic experience, demonstrating a solid foundation in therapeutic techniques.

• Current CPR certification to ensure the safety of our patients during therapy sessions.

Preferred Experience

While two years of outpatient orthopedic experience is required, any additional experience in home care or with specialized patient populations will be viewed favorably. Familiarity with comprehensive patient evaluation techniques, as well as knowledge of documentation software and rehabilitation protocols, is advantageous.

Compensation and Benefits

The salary range for this position is competitive, offering between USD 100,000 and 120,000 annually, commensurate with experience.
