Work Environment

Key Responsibilities

• Conduct thorough evaluations and assessments of patients, including gathering pertinent medical history and utilizing specific diagnostic tools to inform treatment planning.
• Communicate evaluation results and treatment recommendations clearly to patients and their families, incorporating their input into care goals and objectives.
• Implement individualized treatment protocols based on evaluation findings, tailoring approaches to accommodate each patient's unique presentation and needs.
• Collaborate with multidisciplinary healthcare professionals to ensure comprehensive patient care and to adjust rehabilitation plans as necessary based on ongoing assessments of patient progress.
• Perform timely and accurate documentation of patient evaluations, treatment progress, and discharge planning, ensuring compliance with healthcare regulations and fostering continuity of care.
• Manage your schedule effectively to meet established productivity expectations, participate in team meetings, and contribute to community outreach and education efforts.
• Engage in continual professional development through attending workshops, conferences, and other continuing education opportunities, enhancing your skills and knowledge.
• Prioritize creating a positive patient experience by being flexible, approachable, and responsive to patient needs.
• Encourage team-based problem-solving and participate in initiatives aimed at improving access to care for our communities.

Patient Population / Client Focus

In this role, you will work with a diverse patient population, including individuals recovering from surgery, athletes with sports-related injuries, and those managing chronic pain. The holistic approach at Pathways Physical Therapy emphasizes not just rehabilitation, but improving functional outcomes that allow patients to return to their daily activities. You will play a key role in guiding your patients through recovery, using treatments that may incorporate manual therapy, therapeutic exercises, and patient education.

Qualifications

• Bachelor's degree in Physical Therapy from an accredited institution.
• Current state licensure to practice Physical Therapy.
• Certification in Basic Life Support (BLS) from the American Heart Association.
• Strong interpersonal skills and a commitment to patient-centered care.
• A focus on continuing education and a willingness to stay current with industry advancements.

Preferred Experience

• A minimum of 2 years of experience as a licensed Physical Therapist, preferably in an outpatient setting.
• Familiarity with electronic medical records (EMR) systems.
• Experience in developing therapeutic programs tailored to individual patient needs.
