This full-time, onsite role will primarily focus on providing personalized home care services to patients recovering from orthopedic injuries and surgeries. The successful candidate will play a vital role in enhancing patients' mobility, promoting recovery, and improving overall quality of life.

Work Environment

Working in a home care setting allows our Physical Therapists to provide one-on-one attention and tailored rehabilitation strategies in a comfortable environment for our patients. This onsite position requires traveling to patient residences within North Fort Myers and surrounding areas, which provides our therapists with the opportunity to observe and address mobility challenges in the patients' daily environments. Our working hours are Monday to Friday from 9 AM to 5 PM, offering a stable schedule with weekends off.

Key Responsibilities

• Deliver individualized physical therapy treatment plans, designed to meet each patient's unique goals and physical capabilities.
• Thoroughly assess patients' physical conditions and needs, constantly evaluating treatment effectiveness and making necessary adjustments to care plans.
• Implement a variety of treatment modalities, including manual therapy and therapeutic exercises, to restore motion and enhance functional mobility.
• Educate patients and their families on effective home exercises and healthcare practices to promote long-term wellness.
• Maintain accurate, timely documentation of patient progress and treatment outcomes using EMR software, with emphasis on tools like Web PT.
• Work collaboratively with a diverse healthcare team to ensure comprehensive care that addresses both the physical and emotional needs of patients.
• Foster an encouraging and supportive environment that motivates patients throughout their rehabilitation journey.
• Promote a healthy lifestyle initiative that enhances patients' strength, flexibility, balance, and coordination.

Patient Population / Client Focus

This position focuses on a diverse range of patients, primarily those recovering from orthopedic injuries, post-surgical rehabilitation, and chronic pain conditions. Our client-centered approach prioritizes the specific needs of each individual, ensuring tailored therapeutic interventions that optimize recovery. By providing care in the home environment, we can facilitate healing in a comfortable and familiar setting, which reinforces the success of our treatment strategies.

Qualifications

• Graduation from an accredited Doctorate in Physical Therapy (DPT) program.
• Current, active license to practice as a Physical Therapist in the state of Florida with a clear history and no sanctions.
• Strong proficiency in implementing a wide range of manual therapy techniques and exercise programs tailored to individual patient needs.
• Proven ability to document treatment accurately using EMR systems, with a particular focus on Web PT.
• Continuous commitment to professional development and staying current with best practices and innovations in physical therapy.

Preferred Experience

• A minimum of three years' experience in orthopedic rehabilitation in an outpatient or home care setting is preferred.
• Familiarity with evidence-based practices and a solid understanding of current physical therapy modalities.
• Experience in educating patients on lifestyle changes and wellness strategies that complement rehabilitation.

Compensation and Benefits

We offer a competitive salary ranging from USD 100,000 to 120,000 annually, commensurate with experience.
