Work Environment

This physical therapist role is primarily onsite, as you will be working directly with patients within their home environments. Key Responsibilities

• Conduct comprehensive evaluations of patients recovering from various conditions including injuries, surgeries, or chronic diseases within their home settings.
• Develop individualized treatment plans utilizing evidence-based practices aimed at alleviating pain and enhancing mobility through targeted exercises, manual therapy, and other modalities.
• Track and meticulously document patient progress, adjusting treatment goals and plans based on their evolving needs to ensure optimal outcomes.
• Instruct patients and caregivers on the proper use of assistive devices and therapeutic techniques, while fostering their independence in daily activities.
• Engage in continuous professional development through educational workshops and collaborative learning opportunities with peers and mentors.

Patient Population / Client Focus

Qualifications

• Bachelor's Degree in Physical Therapy from an accredited institution.
• Current state license in Physical Therapy.
• Strong verbal and written communication skills.
• Proficient computer skills for documentation and information management.

Preferred Experience

• At least two years of experience in a physical therapy setting, ideally within home care.
• Possession of a Doctorate of Physical Therapy (DPT) is preferred but not mandatory.
• Current CPR certification.
