As a full-time, onsite practitioner, you will directly support patients in their homes, helping them achieve optimal health and mobility. Work Environment

This position is based onsite with a focus on home care services, allowing you the unique opportunity to work in patients' homes. Key Responsibilities

As a Physical Therapist at Agility Peak, your responsibilities will include:

• Conducting thorough assessments for each patient by consulting with them to understand their physical conditions and medical history.
• Diagnosing movement dysfunctions and crafting individualized treatment plans that align with their specific health goals.
• Guiding patients through therapeutic exercises, ensuring they utilize proper techniques to enhance recovery.
• Utilizing various interventions, such as manual therapy, electrotherapy, and functional tasks to expedite healing.
• Training patients and their families on the safe and effective use of adaptive equipment, such as wheelchairs, walkers, and assistive devices.
• Tracking and maintaining detailed patient records, documenting progress, setbacks, and evaluating treatment efficacy.
• Advising clients and their families on home-based treatment options and exercises to continue rehabilitation outside of our therapy sessions.

Patient Population / Client Focus

Your role primarily involves working with a diverse patient population ranging from young children to elderly adults, each presenting varying levels of physical challenges. This role also offers the rewarding experience of assisting individuals recovering from surgeries, injuries, or chronic conditions, closely tracking their progress and helping them achieve their recovery goals.

Qualifications

To be successful in this role, candidates should possess:

• A Bachelor's Degree in Physical Therapy from an accredited program.
• A valid state license in Physical Therapy.
• Strong verbal and written communication skills to effectively interact with patients, families, and interdisciplinary teams.
• Proficiency in using computer systems for documentation and patient management.
• A highly flexible demeanor to adapt to the dynamic home care environment.

Preferred Experience

While a minimum of 2 years of experience is preferred, we are enthusiastic about welcoming passionate new graduates to our team. Experience in home health care or outpatient settings will be considered a valuable asset.

Compensation and Benefits

This position offers competitive compensation, with a salary range of USD 80,000 – 100,000 per year, commensurate with experience and qualifications. In addition to a robust salary, we provide a comprehensive benefits package that includes:
