Work Environment

This position is primarily onsite, where you will engage with patients in their residential settings throughout Cape Coral and surrounding areas. The home care environment requires adaptability and creativity, as you will encounter a variety of home conditions and patient circumstances. Key Responsibilities

As a Home Care Physical Therapist, your primary responsibilities will include:

• Developing, implementing, and adjusting individualized treatment plans based on thorough assessments and in accordance with physician directives.
• Administering therapeutic exercises, manual therapy techniques, and modalities to promote recovery and rehabilitation, while educating clients on their use.
• Evaluating each patient's home environment to enhance safety and independence, making recommendations for modifications as necessary.
• Supervising Physical Therapy Assistants in the implementation of care plans and ensuring adherence to professional standards of practice.
• Documenting progress and any changes in patient status in compliant clinical records, including OASIS assessments and treatment summaries.
• Collaborating with an interdisciplinary team, participating in case conferences, and contributing insights for holistic treatment planning.
• Providing instruction to patients and their families on therapeutic exercises and self-care techniques to encourage long-term health gains.
• Keeping timely records and complying with branch policies for documentation and orders to reflect accurate clinical and payroll information.
• Promoting an ethical and engaging atmosphere, serving as an advocate for patient needs and preferences.

Patient Population / Client Focus

This role will focus on a diverse patient demographic, including individuals recovering from surgery, those with chronic pain, and elderly patients requiring rehabilitation. The aim is to facilitate optimal recovery and independence, tailoring strategies to meet the functional goals of each unique patient. By working closely with families, the PT will empower clients through education and skill-building, fostering a collaborative effort toward health and wellness.

Qualifications

To be considered for the Home Care Physical Therapist position, candidates must possess:

• A Bachelor's degree in Physical Therapy; candidates holding a Doctorate in Physical Therapy (DPT) are preferred.
• A valid Florida license to practice as a Physical Therapist, with evidence of current standing.
• Current CPR certification, ensuring readiness for any emergency situations.
• A valid driver's license with appropriate automobile insurance, as travel between patient sites is essential for this position.

Preferred Experience

We are looking for candidates with at least two years of experience in a physical therapy setting, preferably in a home care or acute care environment. Strong candidates will be creative, capable decision-makers who can manage complex patient cases while demonstrating expertise in functional and therapeutic interventions. Experience in working as part of a multidisciplinary team is a strong asset, as is familiarity with federal and state regulations related to home healthcare.
