Work Environment

This role is primarily onsite, delivering care directly in patients' homes across Cape Coral and the surrounding areas. Key Responsibilities

• Conduct thorough examinations and evaluations of patients to identify functional limitations and develop individualized treatment plans.
• Collaborate with other healthcare professionals to ensure a continuum of care, providing consultations and updates to referrals as necessary.
• Engage with patients across all age groups, demonstrating cultural sensitivity and adherence to exemplary customer service standards.
• Implement treatment plans effectively while continually evaluating patient progress and adjusting interventions as required.
• Document patient evaluations and treatment plans accurately, ensuring compliance with all applicable laws, regulations, and professional standards.
• Maintain open lines of communication with the interdisciplinary treatment team, physicians, and other referral sources to ensure a cohesive treatment approach.

Patient Population / Client Focus

Your primary focus will be on patients recovering from orthopedic procedures, injuries, or chronic conditions affecting their mobility and function. You will work with a diverse clientele, tailoring your approach to accommodate various ages, cultural backgrounds, and individual needs, ensuring you provide holistic and effective care.

Qualifications

• Doctorate of Physical Therapy (DPT) from an accredited institution.
• Current State License as a Doctor of Physical Therapy from the Physical Therapy Examining Committee.
• A minimum of two years of related clinical experience in a therapeutic setting.
• Current CPR certification, ensuring readiness for emergencies.

Preferred Experience

While two years of experience is required, candidates with additional certifications in orthopedic therapy or specialization in geriatric care are preferred. Familiarity with electronic health record (EHR) systems is a plus, as is experience working within a home care environment.

Compensation and Benefits
