Work Environment

Our Physical Therapists work in the heart of the Cape Coral community, providing care in patients' homes, thereby ensuring a comfortable and familiar setting for rehabilitation. You will collaborate with a dedicated team of healthcare professionals, including physical therapy assistants and interdisciplinary team members to create tailored care plans that address the unique needs of each patient.

Key Responsibilities

• Direct and implement physical therapy treatment plans aimed at preventing disability, relieving pain, and restoring functional mobility.
• Oversee the work of physical therapy assistants to ensure compliance with the established plan of care.
• Conduct thorough evaluations of clients' functional abilities through diagnostic and prognostic assessments.
• Assess clients' home environments and recommend modifications to improve rehabilitation outcomes and independence.
• Administer manual therapeutic exercises, instructing and motivating clients through both manual and non-manual activities.
• Deliver additional modalities such as traction and massage as prescribed by a physician.
• Fit and adjust prosthetic and orthotic devices, making recommendations for modifications to enhance client functionality.
• Document each patient's progress and changes in condition, offering insights for potential adjustments to the treatment plan.
• Prepare comprehensive clinical records, including evaluations, progress notes, and discharge summaries, ensuring documentation is in line with agency policies.
• Engage in interdisciplinary case conferences, facilitating discussions that evaluate and enhance the holistic treatment approach for clients.
• Provide educational training to clients and their families on therapy techniques and device usage.
• Complete functional assessments, ensuring adherence to best practices in utilization of community resources for enhanced patient support.
• Maintain open communication with clients, families, and healthcare teams, ensuring alignment in care objectives.
• Fulfill additional duties as assigned to support organizational goals and enhance client care outcomes.

Patient Population / Client Focus

Your role as a Physical Therapist will include working with a diverse range of clients who may be recovering from surgeries, managing chronic conditions, or faced with disabilities affecting their daily activities. Qualifications

• Doctorate of Physical Therapy (DPT) from an accredited educational program.
