Key Responsibilities\n

• Conduct in-depth assessments of senior clients’ needs to identify and understand their mental health challenges.

• Develop personalized treatment plans and strategies to help clients navigate their mental health journey, including crisis intervention and case management.

• Provide individual counseling or psychotherapy sessions for adult and senior clients, utilizing evidence-based therapeutic modalities.
\n
• Research and refer clients to additional resources and support systems available within the community, ensuring comprehensive care.
\n
• Maintain accurate and timely documentation of client interactions, treatment plans, and progress notes in accordance with state regulations.

• Work as an Independent Contractor, managing workloads effectively while remaining aligned with Caring Heart's mission and guidelines.

Patient Population / Client Focus\n

The primary client population consists of adults and senior individuals who face various mental health issues, including anxiety, depression, trauma, and substance use disorders. A key focus of our practice is supporting clients in navigating detoxification processes and ensuring that they receive holistic care that addresses both their psychological and emotional needs.

Qualifications\n

• A Master’s degree in Social Work (MSW) is required.

• Current and valid state licensure as a Licensed Clinical Social Worker (LCSW).

• Proficiency in Microsoft Office Suite (Word, Excel, PowerPoint) and ability to maintain electronic client records.

• Exceptional communication skills and the ability to quickly build rapport with clients in a remote setting.

• Self-motivated and able to manage responsibilities independently while adhering to organizational guidelines and confidentiality standards.

Preferred Experience\n
