Case Management Coordinator Easterseals MORC is hiring a Case Management Coordinator to help make a difference and become part of something bigger than yourself. Benefits Low‑cost Dental/Health/Vision insurance Dependent care reimbursement, and up to 5 days paid FMLA for maternity, paternity, foster care and adoption Generous 401K retirement plan Paid Leave Options Up to $125 bonus for taking 5 days off in a row 10 paid holidays and 3 floating holidays Wellness Programs PSLF (Public Service Loan Forgiveness) Employer Bonuses and extra incentives to reward hard work & dedication Mileage reimbursement in accordance with IRS rate Free financial planning services through our partnerships with the LoVasco Consulting Group and SoFi Student loan repayment options Pet Insurance Qualifications Must be a QMHP in accordance with Medicaid Provider Manual Guidelines Possess specialized training (including fieldwork and/or internships associated with the academic curriculum where the student works directly with persons receiving mental health services) OR one year of experience in treating or working with a person who has mental illness Be a human services professional with at least a bachelor’s degree in a human services field Duties and Responsibilities Demonstrate the ability to engage individuals in a welcoming, hopeful, empathic manner regardless of disability or phase of recovery Screen for co‑occurring disorders and recognize diagnostic criteria used to identify substance abuse or dependency Assess and evaluate the needs of individuals and continue caseload contact to develop goals; identify stage of change and use interventions consistent with stage of treatment; assist clients in developing strength‑based, stage‑wise, treatment‑matched person‑centered plans that address identified needs; complete referral forms, clinical and legal documentation necessary to obtain community‑based services for clients as well as collaborate and communicate with other SA/MH community partners Advocate and provide options for supports and services as needed and seek out and develop community resources to meet the needs of assigned individuals for activities and support services Easterseals MORC was awarded Metro Detroit and West Michigan 101 Best & Brightest Companies to Work For! #J-18808-Ljbffr
