Work Environment

The Rehabilitation Center fosters a collaborative and patient-focused atmosphere. Our team consists of skilled professionals who are dedicated to providing exceptional care to every individual. Located in the heart of San Jose, California, our facility is easily accessible and designed for efficiency and comfort, enhancing both the staff and patient experience. You will work on-site Monday through Friday from 2 PM to 8 PM, allowing for a balanced work-life schedule.

Key Responsibilities

• The Physical Therapist is responsible for providing high-quality, patient-centered care through the management of therapeutic processes for a designated group of patients in collaboration with physicians and the multidisciplinary care team.
• You will conduct thorough patient assessments to evaluate functional status and develop individualized care plans tailored to meet specific patient goals and improve overall engagement.
• Deliver and oversee the primary aspects of patient therapy throughout the continuum of care, ensuring adherence to best practices.
• Perform evaluations and interventions within the professional code of ethics and in alignment with legal standards.
• Engage actively in the therapeutic process by instructing patients, families, and caregivers in therapeutic exercises and the use of assistive devices to encourage recovery and independence.
• Coordinate caseload management, assess daily staffing needs, and ensure efficient assignment hand-offs for optimal patient flow.

As a Physical Therapist, you will be working with a diverse patient population that includes individuals recovering from surgeries, pediatric cases, and those needing rehabilitation due to chronic pain or injuries. Qualifications

• A Bachelor's Degree in Physical Therapy or a Doctorate in Physical Therapy (DPT) is required.
• Current and valid state license to practice as a Physical Therapist.
• Basic Cardiac Life Support (BCLS) certification must be obtained within 30 days of employment commencement.

Preferred Experience

• While no experience is required, previous experience in a rehabilitation setting will be considered an asset.
• Understanding of therapeutic practices and knowledge of rehabilitation equipment is preferred.
