The selected candidate will primarily work from home, supporting patients through a virtual platform, but will occasionally engage with healthcare teams in a community health clinic setting located in Fontana, California. Key Responsibilities\n

• Manage an individualized caseload of patients, conducting thorough assessments to identify social, emotional, financial, and cultural influences impacting their healthcare journey and treatment plan.

• Advocate for patient autonomy and develop effective strategies to overcome psychosocial barriers, ensuring timely transitions across various levels of care.

• Coordinate with multidisciplinary healthcare teams, providing essential education and resources to patients and their families regarding protective services, life care decisions, and advance directives.

• Implement crisis intervention techniques, benefiting patients and families during stressful situations while collaborating with healthcare professionals to facilitate necessary interventions.

• Provide clinical supervision to junior staff, including MSW students and interns, fostering their development while contributing to excellence in practice standards.

Qualifications\n

Compensation and Benefits\n

The salary for this position ranges from USD 120,000 to 130,000 annually, commensurate with experience and qualifications.
