• Act as an advocate for clients; facilitate access to community resources including healthcare, transportation, and social services, ensuring they have the support they need to thrive.

• Conduct regular virtual check-ins with clients, monitoring their progress, and addressing any concerns or changes in their living conditions.

• Maintain meticulous documentation, including assessments, care plans, and progress notes, ensuring compliance with ethical and regulatory standards.

Qualifications\n
