Love Bound Therapy is seeking a dedicated Licensed Independent Clinical Social Worker (LICSW) to join our team, focusing specifically on senior services. Work Environment\n

This role is primarily remote, allowing for a flexible work environment. While you will manage your schedule effectively to meet client demands, occasional in-person visits to clients’ homes or facilities may be required. The typical work hours are Monday to Friday from 8:00 AM to 4:30 PM, with occasional evenings or weekends based on client needs. The successful candidate will be expected to maintain availability for urgent situations, ensuring that we provide continuous, high-quality support to our clients and their families.

Key Responsibilities\n

• Conduct comprehensive assessments of seniors’ psychosocial and emotional needs, taking into consideration their physical health, living situations, and personal histories.

• Develop personalized care plans in collaboration with clients and their families to address specific challenges and goals.
\n
• Provide individual and family counseling to support clients in navigating complex life transitions, such as moving to assisted living or coping with loss.
\n
• Advocate for client rights and access to community resources, ensuring that seniors receive the necessary services and support.
\n
• Engage with interdisciplinary teams, including healthcare professionals and caregivers, to coordinate care that encompasses all aspects of a client's well-being.

• Regularly monitor clients’ progress through scheduled visits, documentation of care, and ongoing communication with family members and caregivers.

• Facilitate family meetings to address concerns, improve communication, and resolve conflicts regarding care decisions.
\n
• Educate clients and their families about social resources, including support groups and community programs aimed at enhancing social connections and engagement.

• Maintain accurate and timely documentation of all client interactions, assessments, and care plans using electronic health record (EHR) systems.

• Stay abreast of current trends and regulations in social work and geriatrics to ensure compliance and best practices in service delivery.

Qualifications\n

• A Master’s degree in Social Work from an accredited institution.

• Current and valid state licensure as a Licensed Independent Clinical Social Worker (LICSW).

Compensation and Benefits\n
