About the Position

Position Overview

The Nurse Practitioner will provide comprehensive outpatient primary care services to individuals and families, focusing on prevention, wellness, chronic disease management, and patient education. The role emphasizes team-based care, leadership within the practice, and delivering high-quality, compassionate healthcare.

Schedule & Assignment Details

• Full-Time Permanent Position (1.0 FTE)

• Monday – Friday, 8:00 AM – 5:00 PM

• 40 hours per week

• 1:11 call rotation (no routine weekends)

• Outpatient Primary Care setting

• Collaborative, multidisciplinary medical group

Key Responsibilities

• Provide comprehensive outpatient primary care within APRN licensure standards

• Evaluate, diagnose, and manage acute and chronic medical conditions

• Emphasize health promotion, disease prevention, and wellness initiatives

• Perform patient assessments, develop treatment plans, and manage follow-up care

• Prescribe medications and therapies in accordance with DEA and licensure guidelines

• Educate patients and families to support informed healthcare decisions

• Collaborate closely with physicians, nurses, medical assistants, and administrative staff

• Identify patient education needs and deliver targeted health counseling

• Participate in practice quality initiatives and ongoing professional development

• Maintain accurate and timely clinical documentation

Requirements

Qualifications & Requirements

• Master’s Degree from an accredited Adult Nurse Practitioner program – Required

• Current Vermont APRN License (or ability to obtain) – Required

• National Certification as Adult, Family, or Pediatric NP – Highly Preferred

• DEA License – Required

• Healthcare Provider CPR Certification – Required

• Qualified for medical staff credentialing – Required

• Clean malpractice history – Required

• Two years of nursing experience prior to advanced degree – Highly Preferred

• Most recent NPDB Self-Query Report (within 30 days) – Required at presentation
