The Nurse Practitioner reports directly to the Nurse Practitioner Lead Clinician. The Nurse Practitioner position is a role that requires management of a full time patient load. Nurse Practitioner will be expected to see patients in all settings. This includes, but is not limited to, skilled nursing facilities, assisted living facilities, patients homes, etc. The Nurse Practitioner is responsible for implementing the most appropriate treatment protocols for all patients under their care. The Nurse Practitioner is responsible for accurate and appropriate charting and documentation of all care and treatment provided. The Nurse Practitioner is responsible for all tests, diagnostics, etc. that are ordered by the Nurse Practitioner. The Nurse Practitioner will work directly with the Regional Clinical Director to work towards consistent improvement on clinical operations, and ultimately working towards always improving the quality of care ReSkin Medical patients receive. The Nurse Practitioner is responsible for facilitating timely and appropriate communication with all patients, healthcare partners, etc. The Nurse Practitioner will work closely with the Clinical Management Team to meet all requirements for chart reviews, audits, Quality Assurance items, etc. The Nurse Practitioner will maintain ReSkin Medical’s established culture with all communications and interactions. The Nurse Practitioner will maintain all of ReSkin Medical’s established and documented Protocols and Procedures. RESPONSIBILITIES Facilitating all patient visits for when they are scheduled Meeting all charting and documentation requirements set forth by ReSkin Medical Attending all scheduled meetings, one on ones, chart reviews, etc. Responding to all communications, which include, but are not limited to, phone calls, text messages, emails, etc., within a timely manner, which does not surpass more than one hour Complete all required and assigned educational/continuing education competencies Adjustments to the job duties are at the discretion of ReSkin Medical Signing and lock chart notes within the determined time frame Sending orders within the determined time frame Maintain all required certifications, credentialing, licensure, etc. to practice within the state of operation The Nurse Practitioner is responsible for requesting assistance, guidance, recommendations, consultations, etc., when needed. The Nurse Practitioner is responsible for identifying when they are outside of their scope and require assistance. The Nurse Practitioner is required to obtain a ReSkin Medical approved wound care certification, if not already wound care certified, within one year of their start date with ReSkin Medical. The approved certifications are: WCC, CWS, WOCN. JOB REQUIREMENTS Board-Certified Nurse Practitioner (ANP, AGNP, FNP, ENP, or ACNP) Masters Degree from an accredited Nurse Practitioner program Highly recommended certifications (WCC, CWON, or CWS) At least one year of direct wound care experience At least one year experience as a Nurse Practitioner Prefer experience in Wound Care, Home Care, Emergency Department or Intensive Care Units Must be able to work independently Must be willing to learn and demonstrate the ability to communicate well with family, patients and other team members Must have clear verbal communication skills Consults with definitive diagnostic results; maintains accurate records Reviews medical history, allergies, symptoms, and current medications Prescribes medications to patients and provides proper dosage and administration instructions Excellent analytical and problem-solving skills Will have a set of scheduled hours with routine and new patients; punctuality is critical This position requires travel in your personal vehicle to see patients in their home setting. Mileage is reimbursed. Company reimbursement for successful Wound Care certification test is available. TASKS Knows, follows, and understands the clinical and administrative protocols and procedures in place. This position requires a full and comprehensive understanding of all established and documented ReSkin Medical Protocols and Procedures. Demonstrates consistent, excellent communication with patients, team members, and care partners. The Nurse Practitioner often coordinates care with other parties, including other clinicians and staff members and participating care partners. The Nurse Practitioner needs good communication skills for service recovery where necessary. Possesses knowledge of advanced wound care principles and application through didactic learning and hands-on experience with patient care. The Nurse Practitioner will preferably have worked with wounds or in a Rounding capacity for a minimum of 1 year before accepting a Nurse Practitioner role. Is certified in wound care as a specialty OR is actively pursuing certification in wound care as a specialty. Acceptable certifications include WOCN (Wound, Ostomy & Continence Nursing, CWS (Certified Wound Specialist), and WCC (Wound Care Certified). Job Type: Full-time, Monday through Friday. No weekend shifts! If wound care certified, the employee could be eligible for a pay raise.
