Key Responsibilities\n

• Conduct thorough psychosocial assessments to evaluate the emotional and mental health needs of hospice patients and their families, considering both strengths and areas of concern.

• Deliver individual, family, and group therapy sessions utilizing evidence-based therapeutic approaches, such as cognitive-behavioral therapy (CBT) and trauma-informed care.

• Create and maintain detailed, individualized treatment plans that outline specific goals and interventions tailored to each client’s unique circumstances.

• Collaborate closely with interdisciplinary teams, including physicians, nurse practitioners, and psychologists, to facilitate integrated, client-centered care.

• Provide crisis intervention as necessary, helping clients develop safety plans and manage acute emotional distress effectively.

• Lead group therapy sessions, facilitating psychoeducation classes and support groups that foster a sense of community and shared experience among patients and families.

• Maintain comprehensive documentation of client interactions and treatment plans, adhering to ethical and legal standards to ensure confidentiality and compliance.

Patient Population / Client Focus\n

The ideal candidate will work primarily with hospice patients and their families, addressing complex emotional issues related to life-limiting illnesses. Qualifications\n

• A Master’s degree in Social Work from an accredited program is mandatory.

• An active state license as a Licensed Clinical Social Worker (LCSW) is required for practice.

• A minimum of two years of experience in mental health counseling, preferably within an outpatient or hospice setting.

Compensation and Benefits\n
