We are seeking a Case Manager who is adaptable, solution-focused, and comfortable working in a fast-paced, high-needs environment.
What You'll Do:- Manage a caseload of approximately 20-25 participants, providing short term, goal-oriented case management
- Complete intake assessments and develop individualized service plans focused on housing navigation, healthcare access, mental health support, income stability, and life skills
- Meet with participants regularly to track progress, address barriers, and support rapid stabilization
- Coordinate and refer participants to community resources (DPSS, SSI, healthcare providers, food assistance, housing programs, etc.)
- Respond to urgent and crisis driven client needs while maintaining firm, professional boundaries
- Support participants in developing routines, problem solving skills, and readiness for permanent housing
- Collaborate closely with on-site staff to ensure program compliance, safety, and continuity of care
- Participate in team meetings and trainings as required
- Strong ability to function effectively in an on-site, high traffic environment with frequent interruptions
- At least 2 years of experience working with vulnerable populations, preferably in interim housing, homelessness services, shelters, or social services
- Clear communication, strong documentation skills, and sound judgment
- Emotional resilience and the ability to manage crisis oriented interactions
- Education in social work, human services, or a related field is preferred but not required
