Description

• Conduct thorough patient evaluations in both inpatient and outpatient environments and create tailored treatment plans
• Implement therapy interventions aimed at improving mobility, strength, balance, and overall functional ability
• Collaborate with a multidisciplinary team of nurses, physicians, case managers, and rehabilitation specialists to ensure optimal patient outcomes
• Guide patients and their families with insights on therapeutic goals, discharge planning, and home-based exercises
• Maintain comprehensive documentation of evaluations, treatments, and patient progress within compliance parameters
• Engage in team rounds and interdisciplinary meetings to foster a holistic approach to patient care

Education

• Doctorate in Physical Therapy (DPT) or an equivalent degree from a recognized program

Certifications

• State licensure as a Physical Therapist
• CPR certification desired
• Additional specialized certifications (neurological, geriatric, etc.) are beneficial but not mandatory

Skills

• Preferred experience in a clinical setting
• Capability to manage patients with detailed medical and mobility needs
