JOB SUMMARY:

The Qualified Mental Health Professional (QMHP) is part of a multi-disciplinary team rendering care and services to persons receiving services from Heartland Center for Behavioral Change (HCBC). The QMHP determines eligibility, conducts assessments, and provides a multi-axis diagnosis. They identify needs and indicate the most appropriate level of care for the person. The QMHP may also provide direct service to clients, crisis assistance, and/or refer clients for appropriate mental health services. All services are provided in compliance with the requirements of the Department of Mental Health. Work is performed under the administrative supervision of the Program Manager and reviewed through direct observation, reports, and supervision.

ESSENTIAL JOB DUTIES, EXAMPLES OF WORK:

Work may include, but is not limited to, the following examples:

1. Conducts diagnostic assessments and participates in direct care coordination, treatment plan development, as well as rendering direct care as appropriate.

1. Participates with designated team members to assistin the development of treatment plans, transition plans, safety plans, and discharge plans.

1. Provide direct services, including individual counseling, group counseling, group education, family education/therapy with an emphasis on addressing trauma and co-occurring disorders.

1. Makes referrals for necessary services utilizing community resources.

1. Responds to client crisis, including making arrangements for appropriate crisis oriented clinical interventions.

1. Develops and maintains effective communication and professional, cooperative working relationships with state officials, staff members, stakeholders, and public.

1. Maintains the confidentiality of client information.

1. Adheres to established ethical and behavioral standards of conduct and continuing professional development.

1. Serves as professional representative of the agency in appearance, demeanor, knowledge and application of that knowledge and in work attitudes and work habits.

1. Performs other related work, as assigned.

EXAMPLES OF KNOWLEDGE, SKILLS AND ABILITIES:

• Thorough knowledge of social work, behavioral sciences and allied disciplines involved in the evaluation, care and rehabilitation of persons suffering from mental illness or emotional disturbance
• Knowledge of individual, group and family psychodynamics as they relate to addiction
• Considerable knowledge of the social bases of behavior, such as the psychology of interpersonal relationships, group processes, and organizational and systems theory
• Considerable knowledge of individual differences, such as personality theory, human development, and abnormal psychology
• Thorough knowledge of diagnostic criteria and application of the DSM IV-R
• Knowledge of community resources used in the provision of services
• Knowledge of State regulatory requirements as it relates to practice of social work and substance use disorder
• Skill in resolving conflicts and negotiating with others
• Ability to collect and analyze information in making diagnostic decisions concerning a client’s treatment
• Ability to act decisively when necessary to protect clients; to make judgments about crisis and carry out appropriate course of action
• Ability to establish a professional and therapeutic rapport with clients; to assist, and exercise tact, patience, respect and understanding of others
• Ability to recognize symptoms of substance use disorder and to elicit information from individuals necessary for therapeutic interventions
• Ability to work effectively with persons from diverse cultures and backgrounds and to respect individual differences
• Ability to maintain organized client records in with certification requirements

• Ability to recognize potential ethical problems and address in ethical manner
• Ability to plan and organize working time effectively
• Ability to work with autonomy or with minimum direction
• Ability to motivate clients in recovery process
• Proficiency in use of computers and knowledge of basic software

KEY PERFORMANCE INDICATORS

Services meet billable hour expectations of 300 hours per quarter as measured in Care Logic and quality documentation scores are at 80% or greater. All training is completed as required.

CLINICAL:

1. Assessment reports are completed within one business day of assessment based on signature and status inCarelogic100% of time.

1. Diagnosis is completed within one business day based on signature and status inCarelogic100% of time.

1. Treatment plans are written, signed and completed one business day based on signature and status inCarelogic100% of time.

1. Individual progress notes are completed within one business day based on signature and status inCarelogic100% of time.

EXPERIENCE AND EDUCATION:

The following education and experience qualifications are the minimum requirements used to admit or reject applicants for consideration.

1. A physician, psychologist, clinical social worker, professional counselor, or nurse practitioner licensed under Missouri state law, acting within the scope of practice for their discipline

AND

1. Has at least one year of full-time equivalent experience in the treatment of behavioral health disorders.

NECESSARY SPECIAL QUALIFICATIONS:

• All candidates will be required to submit to a background screening. No candidate may be considered for employment who is under the supervision of the Federal, State, County or Municipal Corrections Department nor have any pending felony or misdemeanor charges other than minor traffic charges.
• All candidates for employment must meet the established minimum qualifications for employment as identified by the applicable regulatory agencies.
