Overview
Physical Therapist – Home Health, Hayward, California. This travel, temporary assignment starts August 1, 2026 and runs for 13 weeks, with guaranteed 30 hours per week. Weekly pay ranges from $1,776 to $1,790. Shifts available: Sunday–Thursday or Tuesday–Saturday within a Skilled Nursing Facility setting. This open role focuses on clinical assessment and therapy planning for residents in a setting that emphasizes restorative care and functional independence.

- Shift options: Sun–Thu or Tue–Sat Responsibilities
- Conduct PT evaluations and create individualized treatment plans for residents in a SNF setting.
- Provide evidence-based therapeutic interventions to improve mobility, strength, balance, gait, and functional independence.
- Document assessments, treatment plans, progress notes, and discharge summaries in compliance with regulatory and facility standards.
- Collaborate with nursing, therapy assistants, and other disciplines to coordinate care and support safe transitions between care phases.
- Reassess progress regularly and adjust goals and interventions to optimize outcomes; educate patients and families on home programs and safety. Requirements
- Active Physical Therapist license in California or eligibility to obtain licensure.
- Degree from an accredited physical therapy program (DPT or equivalent).
- Experience in skilled nursing facility and/or home health settings preferred.
- Ability to work in a travel assignment and navigate varying schedules; strong documentation and communication skills.
- Must be able to work in the specified facility setting and comply with patient privacy and safety requirements. Benefits and Support
