Position Overview

Baylor Physical Therapy is looking for a dedicated and experienced Physical Therapist to join our dynamic team in Winter Haven, Florida. Work Environment

This position is onsite, providing essential rehabilitation services in the comfort of patients' homes in and around Winter Haven. The Physical Therapist will work closely with clients in a familiar environment, allowing for tailored treatments that effectively consider their daily living conditions and personal comfort. You will collaborate with an interdisciplinary team and play a critical role in enhancing the patient's recovery experience while ensuring a holistic approach to their health.

Key Responsibilities

• Direct and manage physical therapy treatment plans aimed at preventing disabilities, relieving pain, and restoring function in accordance with each client's plan of care.
• Evaluate and assess the functional status of clients through diagnostic and prognostic testing, determining the best path for rehabilitation.
• Implement and oversee physical therapy treatments, including manual therapeutic exercises and educating clients and families on non-manual exercises.
• Monitor the effectiveness of treatment plans, observing and recording changes in clients' conditions and collaborating with physicians and case managers to adjust plans as necessary.
• Participate in care plan development, including documentation of initial evaluations, OASIS assessments, progress notes, and discharge summaries for compliance and clinical accuracy.
• Ensure that home environments are conducive to rehabilitation by evaluating space conditions and making appropriate recommendations aimed at improving the client's independence.
• Collaborate with Physical Therapy Assistants, providing supervision and support to ensure effective implementation of treatment plans.
• Actively engage in interdisciplinary team conferences to address comprehensive care solutions for clients.
• Maintain open lines of communication with clients' families, offering education about therapy devices and supporting their involvement in the rehabilitation process.
• Stay up to date with best practices in physical therapy, participating in continual learning opportunities to maintain high standards of care.

Patient Population / Client Focus

You will work with a diverse client population, including seniors facing mobility challenges, individuals recovering from surgery, and patients with chronic pain or disability. Your approach will be both compassionate and clinical, ensuring that you meet each client where they are in their healing journey while promoting their independence and well-being.

Qualifications

• Doctor of Physical Therapy (DPT) degree from an accredited institution.
• Current and valid Physical Therapy licensure in the state of Florida.
• Minimum of two years of experience in a physical therapy role, particularly in home care or acute care settings.
• Current CPR certification and a valid driver's license with reliable transportation.
• Ability to communicate effectively with patients, families, and healthcare teams, demonstrating strong interpersonal skills and empathy.
• Adaptability and problem-solving skills vital for meeting the diverse needs of clients.
• A commitment to continuous professional development and clinical excellence.

Preferred Experience

• Experience working with multidisciplinary teams in a home healthcare setting.
• Proficiency in utilizing electronic health record systems for documentation purposes.
• Experience developing and implementing patient-centered care plans.
• Understanding of various rehabilitation techniques and tools, including prosthetics and orthotics.

Compensation and Benefits

We offer a competitive annual salary ranging from USD 100,000 to 120,000, commensurate with experience and qualifications.
