- Employees, and their families if elected, can participate in medical insurance (full time and ACA eligible), dental (full time), vision (full time) and group life (employee only coverage - full time)
- We offer a combined paid time off (PTO) policy which incorporates state paid sick leave with company paid time off at an accrual rate of 0.0607 per hour worked, equivalent to 120 hours of PTO at 2,080 hours worked in a year. The PTO accrual rate increases after two years of service and again after five years of service
- We offer 6 Paid Holidays (8 hours - full time, 6 hours - part-time per event, excluding on-call staff) and 1 Personal Holiday of Choice per year (excluding on-call staff)
- Retirement planning is encouraged through our 403(b) plan that includes a generous 100% company match on the first 4% of earnings an employee contributes. There is a 5-year vesting schedule for the company match, and minors are ineligible for the company match

- Perform comprehensive assessments and develop individualized care plans based on physical, emotional, and functional needs
- Assess and evaluate therapeutic, rehabilitative and/or functional status; participate in the development of the total plan of care
- Assess for muscle strength, mobility gait, ROM-potential for rehab
- Determine priority needs for therapy patients
- Initiate Physical Therapy program and instructs other personnel and/or family/caregiver members in certain phases of physical therapy with which they may work with the patient, as well as instructing them as to the goals of the Physical Therapy program for the patient by participating in case conferences
- Evaluate the home environment for safety
- Deliver physical therapy services in patient homes, focused on restoring function, mobility, and quality of life
- Educate patients, families, and caregivers on therapy goals, exercises, and techniques for safety and independence
- Collaborate closely with nurses, social workers, chaplains, physicians, and other team members (as needed)
- Monitor and document patient progress, revising care plans as needed; participate in discharge planning
- May train patient in the use of prosthetic and/or orthotic devices
- Strictly follows infection control procedures and safety policies
- Adequately supervise patient care given by assistive personnel such as Physical Therapy Assistants (PTAs) in accordance with regulations and company leadership principles
- Maintain compliance with all federal, state, and organizational policies, including those of the Washington State Department of Health
- Participate in staff meetings, training, and quality improvement initiatives
- Participate in continuing education to stay informed of trends, practice developments and maintain licensing requirements
- Graduate of an accredited Physical Therapy program (DPT or MPT)
- Current Washington State Physical Therapist license in good standing
- 0-2 years' of experience
- Current CPR & BLS certification
- Experience in Home Health and/or Hospice is highly valued
- Driving is an essential function and/or a legitimate business purpose of this position; therefore, a valid driver's license is required as a condition of employment
- Basic proficiency with electronic medical record systems (EMR)
- Compassionate, patient-centered care mindset
- Strong clinical judgment and ability to make ethical decisions
- Ability to work independently and collaboratively
- Excellent communication, prioritization and time management skills
- Respectful and sensitive to patient cultural, spiritual, and emotional needs
- The ability to listen to, understand and communicate information and ideas presented through spoken words and sentences
- The ability to tell when something is wrong or is likely to go wrong. It does not involve solving the problem, only recognizing that there is a problem
- The ability to use your abdominal and lower back muscles to support part of the body repeatedly or continuously over time without "giving out" or fatiguing
- The ability to lift up to 50 lbs
- The ability to function in other people's home setting which can vary widely with the course of a day or week
- Job requires being sensitive to others' needs and feelings and being understanding and helpful on the job
- Job requires being reliable, responsible, and dependable, and fulfilling obligations
