Work Environment

This full-time position offers the unique opportunity to work in a home care setting, allowing you to build meaningful relationships with patients in the comfort of their own homes. Your work schedule will primarily be Monday through Friday, comprising a 40-hour weekly commitment. This flexible environment enables you to tailor your treatment approaches according to the individual needs of each patient, ensuring optimal outcomes.

Key Responsibilities

• Assessment and Treatment Planning: Conduct comprehensive evaluations of patients referred by their physicians, formulating tailored treatment plans that address their specific needs.
• Implementation of Therapeutic Care: Deliver expert physical therapy interventions directly in patients' homes, utilizing a variety of techniques designed to improve their physical condition and functionality.
• Patient Education: Teach patients and their caregivers about exercises, safety, and how to manage their rehabilitative processes effectively, ensuring they are an active part of the recovery journey.
• Team Collaboration: Work alongside a multidisciplinary team of healthcare professionals to maximize patient outcomes through collaborative care efforts, utilizing evidence-based methodologies.
• Customer Service Excellence: Maintain a high standard of customer service, contributing positively to the treatment experience and overall effectiveness of our services.
• Utilization Goals: Meet and exceed minimum utilization goals and other business targets to help drive the overall success of the organization.

Patient Population / Client Focus

In this role, you will specialize in treating a diverse range of patient demographics, including but not limited to orthopedic, pediatric, neurologic, and sports medicine cases. Your expertise will be crucial in helping patients regain function and mobility, thereby enhancing their quality of life. The home care model allows for personalized attention that is often necessary for successful rehabilitation, making this position vital to our patients' healing processes.

Qualifications

• A Doctorate in Physical Therapy (DPT) is required.
• Active and valid State License to practice Physical Therapy.
• A minimum of two years of documented experience in the physical therapy field.
• Certification in Basic Life Support from the American Heart Association.
• Proficient written and verbal communication skills, enabling clear interaction with patients and team members.
• Competent in using word processing and various computer applications relevant to patient care documentation.

Preferred Experience

• Prior experience in home health settings is preferred but not mandatory.
• Familiarity with electronic health record (EHR) software and treatment planning tools will be advantageous.
• Experience collaborating with a multidisciplinary team to provide comprehensive care.

Compensation and Benefits
