This unique remote role is primarily focused on supporting clients within our Hospice Center. Key Responsibilities\n

• Conduct thorough and culturally sensitive biopsychosocial assessments for patients, taking into consideration their mental health, substance use, and medical histories.

• Provide high-quality behavioral health services, including but not limited to individual and group counseling sessions, intake assessments, treatment planning, and crisis intervention.

• Document all clinical interactions, including progress notes and suicide risk evaluations, using our proprietary electronic health record system.
\n
• Act as a resource and liaison for patients and families, offering support, referrals to community resources, and assistance with medication access.
\n
• Collaborate with Quality Improvement teams to gather and analyze data, subsequently using findings to enhance service delivery and patient outcomes.

Patient Population / Client Focus\n

Your primary focus will be on patients undergoing hospice care, a demographic often dealing with complex emotional, social, and psychological challenges. You will facilitate discussions about end-of-life issues, grief, and loss while promoting a supportive, empathetic environment for both patients and their families.

Qualifications\n

• Current and valid state license as a Licensed Clinical Social Worker (LCSW) is required.

• A Master’s degree in Social Work and a minimum of three (3) years of post-licensure experience in a clinical setting.

• Demonstrated knowledge and proficiency in counseling techniques, crisis intervention strategies, and the independent judgment necessary for providing direct therapeutic services.

• Exceptional skills in group facilitation and the ability to plan and lead therapeutic sessions.

Compensation and Benefits\n
