Key Responsibilities\n

As a Licensed Clinical Social Worker, you will report to the Social Work Manager and will be responsible for:

• Evaluating and diagnosing a wide range of psychosocial problems through comprehensive bio-psychosocial assessments, identifying barriers to treatment and facilitating solutions.

• Providing empathetic and ethical outpatient mental health therapy, utilizing both individual and group treatment modalities tailored to each patient’s unique needs.

• Collaborating with medical and support staff to develop comprehensive care plans that enhance treatment adherence and improve patient outcomes.

• Documenting all social work activities meticulously in patient medical records to comply with departmental standards while ensuring the continuous flow of information within the care team.

• Educating patients and their families on care plans, encouraging understanding, acceptance, and active participation in their treatment process.

Qualifications\n

To qualify for this position, you must hold:
