Evening & Weekend Hospice Care Manager (RN)

Gentiva is seeking an Afterhours RN Care Manager passionate about providing high-quality hospice care outside standard business hours. This pivotal role includes assessing patient needs, providing direct in-person care, and working collaboratively with an interdisciplinary team for seamless patient support. The ideal candidate will hold a valid RN license, have strong hospice knowledge, and bring at least one year of nursing experience.
