Key Responsibilities\n

As a Licensed Clinical Social Worker at Crossroad Behavioral Health Services, your daily responsibilities will include:

• Conducting thorough assessments of clients' psychosocial needs, including their emotional, spiritual, and social requirements during their hospice journey.

• Utilizing DSM (Diagnostic and Statistical Manual) criteria for substance use disorders and collaborating with fellow professionals to determine the best level of care based on ASAM (American Society of Addiction Medicine) standards.

• Developing individualized treatment plans highlighting both substance abuse and psychosocial counseling needs.
\n
• Managing the discharge/continuum of care plans to ensure proper transitions and support for clients moving through various levels of care.

• Documenting counseling and educational services, client assessments, and progress in clinical records to maintain accurate and up-to-date client information.

• Maintaining an active caseload while conducting regular case management updates and treatment planning tasks to track client progress effectively.

• Coordinating with family members and external support services to ensure a comprehensive approach to care.

• Facilitating crisis interventions as necessary, utilizing a team-oriented approach for effective outcomes.

Patient Population / Client Focus\n

Your primary focus will be on clients with advanced illness in the hospice setting, encompassing a wide range of ages, backgrounds, and psychosocial needs. You will provide empathic support to patients and their families grappling with grief, loss, and the challenges of impending bereavement. Your expertise in both addiction and grief counseling will be invaluable, as many clients may experience co-occurring challenges that require specialized attention.

The ideal candidate will possess:
