Key Responsibilities\n

• Conduct comprehensive psychosocial assessments by interviewing patients, observing their interactions, and analyzing relevant medical records to identify emotional, social, and environmental factors that may impact recovery.

• Deliver educational sessions to patients and families regarding available community resources and post-rehabilitation services that can help sustain recovery post-discharge.

• Engage in discharge planning processes, facilitating smooth transitions for patients moving to home care or other care settings, confirming they have adequate support and resources available.

You will work primarily with patients in a hospice setting who are experiencing the complexities of rehabilitation due to conditions like stroke, spinal cord injury, orthopedic surgeries, and various neurological disorders.
