Position: Home Health RN Case Manager Role & Responsibilities Provide personalized, home-based care to patients recovering from illnesses, injuries, or managing chronic conditions. Collaborate with physicians, oversee LPNs, CNAs, and aides to deliver comprehensive patient care. Conduct patient assessments, document progress, and communicate changes to healthcare providers. Educate patients and families on care plans, medications, therapies, and safety measures. Coordinate patient discharge and ensure care goals are achieved. Qualifications & Skills Active, unrestricted RN license in Michigan. Minimum of one year of clinical nursing experience. Current CPR certification. Reliable transportation, valid driver’s license, and insurance. Key Attributes: Autonomy, strong communication, patient-focused care, teamwork, and dedication to quality service. #J-18808-Ljbffr
