Work Environment

This role is positioned in a home care setting, allowing our therapists to provide personalized, one-on-one care to patients across the San Bernardino area. You will enjoy a flexible Monday to Friday schedule, working from 9 AM to 5 PM, which allows for work-life balance and the opportunity to make meaningful connections with patients and their families.

Key Responsibilities

As a Physical Therapist, your primary duties will include:

• Administering direct patient care in accordance with the prescriptions from healthcare providers, ensuring that all treatments are tailored to the individual's needs.
• Performing thorough evaluations using appropriate assessment techniques to determine patient progress and develop effective treatment plans.
• Designing and implementing personalized treatment programs that align with established rehabilitation goals.
• Monitoring patient responses and modifications to treatment based on real-time observations of their tolerance and status during sessions.
• Documenting all patient interactions and maintaining comprehensive records in line with departmental policies to ensure continuity of care.
• Engaging with interdisciplinary team members for collaborative care and participating in family meetings to discuss patient management strategies.
• Participating in quality improvement initiatives by reviewing patient charts and outcomes to enhance service delivery.
• Responding to emergency procedures and protocols to maintain patient safety and well-being during treatment.

Patient Population / Client Focus

This role focuses on a diverse range of patients, including individuals recovering from surgery, injuries, chronic pain conditions, and neurological disorders. Qualifications

To be considered for this position, candidates must possess the following qualifications:

• A Bachelor's degree in Physical Therapy from an accredited institution.
• A valid state license to practice physical therapy in California.
• A minimum of 2 years of experience in a physical therapy setting, preferably with home care experience.

Compensation and Benefits

We offer a competitive salary range of $110,000 - $120,000 annually, commensurate with experience.
