NOR Healthcare Systems seeks a Chronic Care Manager for Population Health and Value Based Care to coordinate patient care across ambulatory and facility settings. The role focuses on improving chronic care outcomes through a proactive, collaborative approach with care teams and leadership. The ideal candidate is a California RN with strong communication and problem-solving skills, BLS and ACLS, and experience with care management software such as Allscripts.
