Your future role at a glance Location: Jacksonville, FL Facility: Ascension St. Vincent's Riverside Hospital Schedule: Day Shift | Full-time How you'll make an impact in this role Orchestrate the patient journey by leading interdisciplinary teams to design and execute seamless care plans from admission through discharge. Champion patient-centered care by integrating family values and personal preferences into every stage of the clinical process. Drive clinical outcomes through proactive assessments and continuous monitoring to ensure every intervention is effective and purposeful. Eliminate barriers to recovery by advocating for essential resources and resolving obstacles that impede high-quality care. Serve as a strategic lead on medical documentation and post-discharge planning to ensure long-term patient success and compliance. What minimum qualifications you'll need Licensure / Certification / Registration: Certified Case Manager credentialed from the Commission for Case Manager Certification (CCMC) preferred. ACM also accepted. Certified Social Worker preferred. Education: Bachelor's degree in Social Work required. Work Experience: 3 years of experience preferred.
