Work Environment

This role will be performed onsite, allowing for direct interaction with patients in a home care setting. Key Responsibilities

• Conduct thorough evaluations, including reviewing pertinent patient history and performing specialized assessments to formulate effective treatment plans based on evaluation findings.
• Communicate clearly with patients and their families regarding evaluation results and treatment goals, actively involving them in the goal-setting process to ensure tailored care.
• Implement and adapt treatment protocols proficiently, taking into account the latest evidence-based practices and the evolving needs of patients.
• Collaborate with multidisciplinary healthcare professionals to optimize patient care, documenting any changes in the patient's condition that may necessitate adjustments to the treatment plan.
• Manage discharge planning and follow-up care efficiently, ensuring accurate and timely documentation that reflects the course of patient care.

Qualifications

Compensation and Benefits
