Key Responsibilities\n

• Provide educational resources and emotional support to patients and their families regarding available community programs and post-rehabilitation services.

• Participate in comprehensive discharge planning, helping ensure a smooth transition for patients returning home or moving to other care settings, fostering independence and self-sufficiency.

Patient Population / Client Focus\n

Qualifications\n

Compensation and Benefits\n

This position offers a competitive salary range of USD 75,000 – 100,000 annually, supplemented by a sign-on bonus.
