Key Responsibilities\n

• Facilitate connections to community resources, such as support groups and vocational rehabilitation services, to assist patients in attaining necessary support post-discharge.

• Offer educational resources to patients and families regarding post-rehabilitation care and available community resources to promote successful reintegration into daily life.

• Actively participate in discharge planning, ensuring seamless transitions to home or other care settings, while addressing ongoing psychosocial support requirements.

You will work with a diverse patient demographic, primarily consisting of individuals who are recovering from injuries, illnesses, or disabilities that impact their mental and emotional health. Qualifications\n

• A Master’s degree in Social Work from an accredited program.

Compensation and Benefits\n
