Key Responsibilities\n

• Conduct detailed psychosocial assessments using interviews, observations, and comprehensive medical record reviews to identify the emotional, social, and environmental factors impacting each student's recovery.

• Deliver individualized counseling and emotional support to students and their families, particularly in navigating adjustment issues, grief, and coping mechanisms.

• Collaborate closely with the rehabilitation team to develop and implement individualized treatment plans that address the unique psychosocial needs of each student.

• Assist students in accessing essential community resources, including support groups, vocational rehabilitation services, and housing assistance to ensure a network of support.

• Educate patients and families on available community resources post-rehabilitation to empower informed decision-making regarding ongoing care.

• Participate in discharge planning, facilitating a smooth transition for students from school settings to home or further care settings, ensuring continuity of support.

Compensation and Benefits\n
