Overview PATH (People Assisting The Homeless) is seeking candidates passionate about helping others make a positive change in their lives to join our Time Limited Subsidy team as the Case Manager at the Long Beach location. Since 1984 PATH has connected clients to a continuum of homelessness prevention, street outreach, employment preparation and placement, individualized case management, supportive services, interim housing, and permanent supportive housing. What You’ll Do The Case Manager uses a "whatever it takes" approach to assist clients in breaking the cycle of homelessness. You will support clients through all phases of homelessness and help them access and maintain services across healthcare, social services, interpersonal support systems, and the Coordinated Entry System (CES). You will provide individualized supportive services to help each client develop a plan to address strengths, barriers, health needs, income, and housing goals. Conduct intake and develop an Individualized Service Plan (ISP) addressing housing goals, income, and other personal goals (medical, mental health, substance use, financial resources, vocational, and social support needs). Provide referrals, linkages, information, and support to resources to achieve ISP goals. Complete program-specific assessments (e.g., VI-SPDAT). Evaluate strengths and challenges and conduct a 90-day review with the participant after intake. Maintain documentation standards, complete case notes in a timely manner, and keep participant files current with required documents. Maintain confidentiality of participant files per HIPAA and applicable guidelines. Develop therapeutic and professional relationships with clients and partners to engage and retain clients toward housing goals. Provide crisis intervention and risk assessment to increase safety and support clients in self-directed problem solving and coping, including services for individuals with chronic trauma. Use motivational interviewing and trauma-informed care in service delivery. Maintain up-to-date knowledge of community resources and provide wrap-around services. Preferred Qualifications Two (2) years of experience in healthcare, case management, homeless services, or working with vulnerable populations or a related field. Minimum Qualifications All levels of education and experience are welcome. Mandatory Requirements Employment eligibility verification. Ability to obtain CPR/First Aid training. Complete required items as a condition of hire: Tuberculosis Test, Background Screening, Drug Test. Driving Driving is an essential function of this position.
