Job Title: Registered Nurse (RN) - Start of Care Employment Type: Full-Time, Part-Time, or Per Diem The Start of Care Nurse plays a critical role in initiating home health services for new patients. You will perform comprehensive assessments and develop individualized care plans to ensure continuity and quality of care. Initial Start of Care: $150 per visit 90-Day Post-Operation Visit: $60 per visit Develop and implement individualized care plans in compliance with regulatory standards. Educate patients and families on care procedures, medications, and health management. Coordinate with interdisciplinary teams to ensure seamless transitions and ongoing care. Current and valid RN license in New York State. Strong knowledge of OASIS documentation and regulatory requirements. Ability to travel to patient homes within assigned territory.
