Key Responsibilities\n

• Conduct individual counseling sessions through video conferencing platforms and at our hospice facility, focusing on the emotional and psychological well-being of clients and their families.

• Assess clients’ needs through comprehensive interviews and evaluations, developing personalized treatment plans that adhere to best practices in mental health care.

• Implement and monitor effective interventions, providing support and guidance to clients as they navigate their health journeys.

• Advocate for clients by helping them articulate their personal goals and connect them with necessary resources and services.

• Collaborate closely with nurses, physicians, and other healthcare professionals to ensure a multidisciplinary approach to patient care, sharing insights and feedback to enhance treatment outcomes.

Qualifications\n

• A Master's degree in Social Work (MSW) from an accredited institution.

• Current Licensure as a Clinical Social Worker (LCSW) in the state of Oregon, with a clean disciplinary record.

This full-time position offers a competitive salary ranging from USD 115,000 to USD 130,000 per year, based on experience and credentials.
