Key Responsibilities\n

• Manage a designated caseload of patients in the detox process, performing comprehensive evaluations to identify psychosocial barriers that may hinder safe transitions through the continuum of care.

• Employ various therapeutic interventions tailored to the individual needs of each patient, ensuring that care strategies are culturally sensitive and effective in promoting self-determination.

• Lead multi-disciplinary team meetings, coordinating treatment plans addressing complex psychosocial and medical issues faced by high-risk patients. Provide crisis intervention strategies during times of heightened distress or emergency.

• Facilitate connections between patients, families, and community resources, ensuring that psychosocial needs are duly integrated into discharge plans and follow-up care.
