Key Responsibilities\n

• Manage a designated caseload of patients, utilizing a comprehensive assessment approach to evaluate social, emotional, financial, and cultural factors that impact treatment outcomes.

• Develop and implement tailored treatment plans, providing a range of therapeutic interventions and crisis management strategies to address complex psychosocial issues.

• Lead multidisciplinary care coordination efforts, advocating for the rights and needs of patients and their families, and ensuring high-quality psychosocial support throughout their healthcare journey.

• Facilitate communication and collaboration with case managers and healthcare teams, delivering needed resources and educational tools for families navigating healthcare challenges.

• Supervise and mentor junior staff, interns, and volunteers, developing practice standards that enhance service delivery within the Social Work department.

• Coordinate with community agencies to ensure continuity of care post-discharge, actively preventing hospital readmissions through effective resource management.

• Respond promptly to psychosocial needs, minimizing delays in treatment plans, and contributing to a smooth discharge process for patients.

Your primary focus will be on patients undergoing detoxification treatment, often presenting a range of complex psychosocial challenges. Qualifications\n

• Master’s degree in Social Work (MSW) from an accredited institution.

• Active and unrestricted LCSW licensure in the State of California.
