Home Health Physical Therapist

Job Description

The Home Health Physical Therapist provides skilled physical therapy interventions to Home Health and Hospice patients. This role involves comprehensive patient evaluations, establishing treatment plans, and delivering therapeutic exercises and mobility training to promote independence and improve functional status.

Responsibilities

• Review physician therapy orders and complete comprehensive patient evaluations.
• Establish written treatment plans with short-term and long-term goals.
• Provide therapeutic exercises, stretching, mobility training, and pain management.
• Assess home safety and recommend necessary modifications to prevent falls.
• Monitor patient progress and adjust treatment plans as needed.
• Educate patients and caregivers on treatment plans and home exercise programs.
• Maintain accurate and timely clinical documentation including evaluations and progress notes.
• Collaborate with healthcare professionals to ensure comprehensive patient care.
• Travel to patient homes and various settings to provide therapy services.

Required Experience / Certifications / Licensure

• Current Physical Therapist licensure in the state of Montana.
• Valid driver's license and BLS certification.

Schedule

This position is a day shift, requiring 36 hours of work per week.
