Key Responsibilities\n

• Collaborate closely with primary care providers to identify and manage mental health and psychosocial concerns, acting as a pivotal member of the clinical team.

• Develop relapse prevention and crisis management plans tailored to individual patient needs.

• Engage in outreach strategies to identify and support at-risk individuals, helping them stabilize and maintain their mental health.

• Maintain detailed and accurate records of patient interactions, treatment goals, and progress in our electronic health record system to ensure continuity of care.

• Establish and sustain open communication with primary care providers to keep them informed about patient status, contributing to a cohesive team approach to treatment.

• Support patients in adopting healthier lifestyles, particularly when they are simultaneously managing physical health concerns.

• Equip clients with self-management tools and educational resources to actively engage in their recovery process.

• Provide expert consultation to the clinic's management regarding behavioral health services and potential areas for program evaluation.

• Guide patients in adhering to prescribed medical treatments, assisting them with coping mechanisms for any medication side effects.

Patient Population / Client Focus\n

Qualifications\n

• A Master’s Degree in Clinical Counseling from an accredited institution.

• A current Licensed Professional Counselor (LPC) license in Arizona.

Compensation and Benefits\n
